#23087002
Large underground utility vault in the front yard near the sidewalk, covered by two rusty steel lids. One lid has shifted, leaving a wide gap along the center seam and an open rectangular hole at the end of the seam (see close-up photo). The hole is a tripping / foot-trap hazard, especially for children and pedestrians. The lid does not appear to be seated flush in its frame and its corner bolt does not line up. No lettering is visible on the lids (covered by rust), so I am not sure if this is city water, sewer/storm, or another utility (such as an internet/telecom provider). Please inspect, reseat / secure the lid, and route to the correct utility owner if it is not the City's. A plant pot has been placed over part of the vault temporarily. No water leak or odor is visible from the surface — I have not opened the vault.
Resubmitting opens a fresh form pre-filled with this ticket's location and category. The city dispatches the new report independently.
