URGENT – REPEAT REPORT: Severe Roadway Damage, Recurring Sidewalk Uplift, and Possible Underground Utility Damage from Street Tree I am requesting an urgent on-site inspection at 614 4th Ave, San Francisco, CA 94118. A City street tree immediately in front of my property appears to be associated with severe cracking, uplift, and deterioration of the public roadway and sidewalk. Sections of the roadway are broken, displaced, and uneven, creating potential hazards for vehicles and pedestrians. This is a recurring problem that I have previously reported to the City. Last year, I incurred more than $100,000 in repairs involving underground plumbing and water pipes beneath my garage, along with driveway and concrete replacement. Despite these extensive repairs, the newly replaced sidewalk adjacent to the tree is already beginning to lift and separate again. Multiple plumbers who inspected the property, including my own plumber, have expressed concern that public utility pipes beneath the street may also be undermined or affected in the area of the tree. My primary concern is preventing additional property damage and addressing potential public safety and underground utility hazards before the situation worsens. I am requesting: 1. An urgent inspection by SF Public Works/Bureau of Urban Forestry. 2. An inspection of the damaged public roadway and newly lifting sidewalk. 3. Coordination with SFPUC to evaluate possible underground utility damage. 4. A determination of the underlying cause and necessary permanent corrective measures. 5. A written response identifying the responsible department, inspection findings, and planned corrective action. Please do not close this request based solely on a temporary pavement repair without evaluating the underlying condition. I have extensive photographic documentation of the current damage, previous excavation, and completed repairs. This is a repeat notification of an ongoing condition that may cause further damage to public infrastructure and my recently repaired property. Please provide a service request number and inspection timeframe.
