For some reason at the three-way stop controlled intersection at 30th/Church, Muni has a Muni-only signal. It turns red even when no trains are around, and private vehicles get confused and treat it like a traffic light instead of a stop sign. This happens multiple times a day, typically only resolved when someone honks at the person stopped at the light. Why is there a Muni only signal at a stop controlled intersection? This will come as no surprise - people do not look at the Muni Only sign that is beneath the signal.
